If the request is for a sermon:

2. Date
3. Title of Sermon
4. Quantity of Compact Discs
If the request is for a Bible Study class:
1. Your name and phone number or email address
2. Date of lesson
3. Name of lecture series if known
4. Quantity of Compact Discs
